Effectiveness of diagnostic and active detection of arterial hypertension and blood pressure control in an organized population under martial law in Ukraine – the «Strong Hearts» Program

Abstract

The aim – to evaluate the effectiveness of identifying patients with hypertension using an active diagnosis system and to evaluate the effectiveness of providing care using a medical assistant and using various information services.
Materials and methods. The study included a total of 1425 participants, 61.75 % women and 38.25 % men, who agreed to participate in the active hypertension diagnostics system. The largest proportion was made up of participants aged 30 to 59 years – 67.30 %. 1261 (88.49 %) of the examined underwent laboratory examination, and 11.51 % of the respondents refused to undergo a free laboratory examination. 85.54 % of the examined visited a specialist, and 14.46 % refused a consultation. Drugs and all examinations were free of charge. Less than half (48.49 %) of the patients agreed to treatment.
Results. 54.32 % of the patients were diagnosed with hypertension. The drugs were dispensed through pharmacy chains. 493 patients (71.35 %) applied to receive free drugs within the 2nd visit to pharmacies. At the 3rd visit, 387 patients (56.00 %) received medication, at the 4th visit – 307 patients (44.43 %), at the 5th visit – 193 patients (27.93 %), and at the 6th visit – 88 patients (12.73 %). As a result of the program, all participants, both healthy and with detected hypertension, showed an improvement in adherence to a healthy lifestyle, with improved nutrition, increased physical activity, and a decrease in harmful habits. Constant communication between health care providers and program participants had a positive impact on program participants, and the main wish was more live communication and care calls.
Conclusion. This program is an example of an attempt to build a system of active detection, control, and long-term observation in an organized population and has proven the positive role of others, in addition to doctors, healthcare providers, in identifying and improving the adherence of patients with hypertension to treatment.
